VA center to test GLP-1 drugs for reducing veterans' alcohol cravings

A new clinical trial will test whether a class of drugs called GLP-1 receptor agonists can help veterans with alcohol use disorder. The Asheville VA medical center was chosen as one of the sites for this national study. The brief news note didn’t give many details about timelines or how many veterans will be enrolled, but it signals that researchers are expanding trials of these drugs beyond weight loss and diabetes. GLP-1 receptor agonists are medicines modeled on a natural hormone your gut makes after you eat. That hormone, called GLP-1, tells your body to release insulin and also sends signals to your brain that reduce appetite and slow digestion. Popular drugs in this group include semaglutide (sold as Ozempic and Wegovy) and liraglutide (Saxenda). In plain terms, these medicines change how the body and brain respond to food and reward cues. The idea behind the study is that the same brain pathways GLP-1 affects for appetite may also influence alcohol craving and drinking. Past research has shown hints of reduced alcohol intake in animals and in some small human studies when GLP-1 drugs are used. But evidence is still limited: much of it comes from lab animals or small pilot trials, and results have varied. This new trial at the VA is part of a larger, more rigorous effort to test the drugs in a real-world group — veterans with alcohol use disorder — to see if the effect holds up and is meaningful. This matters because alcohol use disorder is common and hard to treat. If GLP-1 drugs can reduce craving or drinking, they could add a new tool for people who haven’t responded to existing therapies. Veterans may particularly benefit because rates of substance use and co-occurring mental health issues can be higher in that population. A successful result would likely lead to more research and potentially new treatment options available through the VA and elsewhere. There are important caveats. GLP-1 drugs are approved for diabetes and weight management, not yet for alcohol use disorder, so their use for this purpose would be off-label unless regulators approve it. They can cause side effects like nausea, vomiting, diarrhea, and in rare cases more serious issues such as pancreatitis. Long-term safety for using them specifically to treat addiction hasn’t been established. The trial will help clarify effectiveness and risks, but until results are published, we don’t know whether benefits will outweigh harms for people with alcohol problems. Bottom line: The Asheville VA joining a national GLP-1 trial is a promising step toward testing whether these appetite-related drugs could help treat alcohol use disorder in veterans, but solid answers will depend on the trial’s outcomes.
